Please ensure that you complete all sections relevant to your business and return to me with the additional info/documents requested, including your liability insurance certificate. Due to the volume of applications we will be unable to process incomplete applications.
On acceptance of your application we will forward a ref. no. which MUST be quoted when making BACS payments, so our Treasurer can identify your payment. Please do not attempt to make payment until you have this.
